Claude Code 2.1.269 flags fabricated tool calls in `/btw` answers instead of printing them

Side questions asked through `/btw` were producing answers containing invented tool calls and their invented output. The fix tells the side question not to write them and flags any that still appear as not executed. It is a small note in a long release body, but it is a named instance of an agent hallucinating evidence of work it never did, in a surface builders read as authoritative.
