MCP's skills extension ratifies three SEP-2640 decisions from Proposed to Accepted

ext-skills PR #132, merged 2026-09-16, moves three entries in docs/decisions.md to Accepted because the SEP text already treated them as settled: resources/directory/read (2026-06-09, keeping its directoryRead capability-gate amendment), skills/get (2026-07-15), and the 2026-07-16 v1 scope decision that drops archives, uses per-file resources with digests, and makes skills/list plus skills/get required. That pins the v1 skills surface for anyone implementing an MCP skills server. The same day the repo also added a SECURITY.md and reorganized its docs.
